The "Community Care Service Voucher for the Elderly" Scheme, launched by the Social Welfare Department, aims to support eligible elders aged 65 and above to choose recognized community care services according to their personal care needs. The Scheme provides financial subsidies in the form of service vouchers, with the subsidy level determined by the elder’s financial means. Adopting a “money follows the user” approach, the Scheme encourages elders to arrange flexible home-based care, thereby enhancing their autonomy, independence, and quality of life.
Service Objectives:
By providing flexible service package, we can meet the diverse needs of the elderly and offer more comprehensive and suitable community care services. The services aim at facilitating service users to continue ageing in place and maintaining their optimal level of functioning.
Target Group of Services︰
  1. Frail elderly residents* in Tai Po District;
  2. Elderly just discharged from public hospitals;
  3. Elderly who have not received any residential care services or subsidized community care services.

* The elderly persons who have been assessed and recommended for CCS and/or RCS under the Standardised Care Need Assessment Mechanism for Elderly Services, and are waitlisting for subsidised CCS and/or RCS on the Central Waiting List (CWL) for Subsidised Long Term Care Services without any kind of RCS or subsidised CCS being received may submit an application.

Application Procedure:
Service Voucher Fees:
The Social Welfare Department determines the level of subsidy based on the income of the voucher holder and their household members, with reference to the median monthly household income updated quarterly by the government. There are six fee levels for co-payment. Elders are required to contribute between 5% and 40% of the service fees, while the government covers the remaining balance. Elders have the flexibility to use their community care service vouchers to purchase services of varying values according to their needs.
