Service Objectives:
- To implement the concepts of “Ageing in Place” and “Continuum of Care” by providing community care services for the elderly, persons with disabilities and individuals and families in need, in order to maintain and enhance their self-care and cognitive abilities, enjoy a dignified, independent, happy and connected life in a familiar community, and avoid premature institutionalisation; the Centre also provides appropriate support for carers to relieve their caring stress.
Target Service Users:
- Home Support Service Cases:
Elders aged 60 or above living in the community, identified with mild impairment or requiring a higher level of care through a simple standard assessment tool, or recommended for home support service upon completion of the Standardised Care Need Assessment Mechanism for Elderly Services.
- Persons with disabilities
- Individuals and families with social needs
- Frail Cases:
Elders who have been assessed under “the Standardised Care Need Assessment Mechanism for Elderly Services” to be suitable for community or residential care services and require a comprehensive package of home care and community support services; the Centre will also provide support to their carers.
