Community Care Services for the Elderly

Tai Po Integrated Home Care Services Centre
Objectives:
- To implement the concepts of “Ageing in Place” and “Continuum of Care” by providing community care services for the elderly, persons with disabilities and individuals and families in need, in order to maintain and enhance their self-care and cognitive abilities, enjoy a dignified, independent, happy and connected life in a familiar community, and avoid premature institutionalisation; the Centre also provides appropriate support for carers to relieve their caring stress.
Target Group of Services:
- Ordinary Cases:
- Elderly aged 60 or above living in the district
- Persons with disabilities
- Individuals and families with social needs
- Mild Impairment Cases:
- aged 60 or above living in the community and assessed to be of mild impairment by the designated assessment tool
- Frail Cases:
- Elders who have been assessed under “the Standardised Care Need Assessment Mechanism for Elderly Services” to be suitable for community or residential care services and require a comprehensive package of home care and community support services; the Centre will also provide support to their carers.

In addition, we are actively training youth to become new members of the caring profession and introducing new technology into our services, so that more elderly can enjoy a quality life in their old age in the community.
